Compiler killed - no codex unlock (video included

Hi! Like many players recently, I participated in a compiler kill just now (did plenty of damage) and did not receive the codex entry for triage.

don't have the whole fight but here is a clip from the last 10 seconds:

Please provide the following details to add to your report:
- The approximate date and time of when this happened
- Did your crew start the Compiler encounter?
- Did you exfil successfully?
- Did your friends get the skin/Codex from the same run? If so, what are their Bungie IDs?
- Were you missing the skin as well as the codex?

patch notes just came out and they did not address this issue at all.

- The approximate date and time of when this happened : 6/2 - approx 11am EST
- Did your crew start the Compiler encounter? we did not
- Did you exfil successfully? yes
- Did your friends get the skin/Codex from the same run? If so, what are their Bungie IDs? they did not. bungie IDs i can get after the servers come back up.
- Were you missing the skin as well as the codex? skin and codex were missing.
