Incoming Voice Chat Cuts Out

Issue Description: Incoming Team Voice Chat Cuts Out Occasionally, but most often after booting up the game.  All other audio, including voice chat from other apps such as discord or steam voice chat work OK.  My microphone also still works, as players have indicated they can hear me when the issue is occurring.  I will note as well that players are not muted, and I can actually see the indicator lighting up showing they are speaking, but none of the incoming audio comes through.  Note that I've also checked my headphone settings, reinstalled drivers, and tried different headphones, but the issue still occurs. 

Issue Replication: Most often the issue seems to start from the moment I turn on the game.  It doesn't always occur, but it seems to happen maybe 25%-50% of the time.  I haven't found a consistent way to replicate the issue, but it definitely happens reasonably frequently. 

Issue Resolution:  Issue will continue indefinitely once it starts until I turn off my headset, then turn it back on again.  Alternatively I can go to windows settings and toggle my audio device.  The fix for the issue very consistent; every time I reset my headset it begins working OK.

Issue Timeline & Prevalence: I started noticing the issue about midway through season 2 (before the latest Nightfall refresh).  Its possible however it was happening longer as there were many times I'd log on for a night and play 5-6 games solo-filling into trios and thought it was unusual that nobody was saying anything.  I don't know how long it was occurring before I finally noticed it and figured it out.    I have talked about the issue to some teammates, and at least 2 of them confirmed the same thing happened to them after I explained the issue to them and they knew what to look for.  I suspect this may be a broader issue that most players who encounter it are unaware of, with player just assuming that their teammates just have dead mics.
