Didn't get Codex entry for cradle reset

I reset my cradle and got the triage skin, but I did not get the cradle: second thoughts codex entry.https://youtu.be/PRid2eAi-vc

    Thank you for reaching out and for reporting this.

    Please provide the following details to add to your report:

    • The approximate date and time of when this happened
    • Do you have a short clip or a few screenshots showing the exact steps you used to reset the Cradle?
    • Did you use the Remove All Energy option or removed points one track at a time before resetting the Cradle?
    • Does the Cradle: Second Thoughts still show as 0/1 after restarting the game?
