Heat grenade dealt 0 damage upon detonating directly next to a runner

Description of Issue - Threw a heat grenade directly on top of a runner and upon detonation, it dealt no damage. There was no grenade damage indicator and the triage's green shield appeared to be completely intact after I opened fire with my LMG. I returned to that spot later and verified that the geometry doesn't not allow a grenade to slip through the spot I threw, unless I sniped a tiny pixel gap.

Note: After watching my replay, I grew suspicious about the runner I hit with the heat grenade, so I filed a report on them for in-game cheating for a possible damage avoidance cheat. Not sure if this will help with finding the root cause.
Rough Date and Time of issue - May 15, 4:15 EST
Repro steps - Not sure but maybe the death bag blocked the blast somehow. It is hard to tell because I hid around a corner to pull my gun out safely. So maybe, in pinwheel blue side room(?), down a runner, have a second runner approach as you throw a heat grenade, kill the downed runner as the grenade detonates. The only thing I can think of is maybe that triage was already overheated so no heat status was applied as well as damage dealt.
