Graphical issues in the codex

- Description of Issue - When browsing through codex collectibles I noticed that one entry in the Dire Marsh section is bugged. The preview icon shows a brain in some sort of black cube which is not visible when actually inspecting the entry. A similar issue is also present when inspecting the "Synapse Cube" item in the loot tab.
- Repro steps - Codex => World => Dire Marsh => Collectibles => Cerebral Record

I'd also like to add that every item in the Loot => Acquired Items tab of the codex has bugged sections of text when inspected. The text describing sources + usage flashes and shuffles around in a way that doesn't look intentional as it's very difficult to read. It seems to only occur if an item has more than one source or usage.
