Having same issue with all factions. My rerolls have been bugged going on 2 weeks.

3. Codex Challenge "Hypocritical Oath" Not Unlocking

So, I randomly completed the challenge a day or two ago. Not sure how, because I don't recall rezzing and hitting a hostile at the same time in my recent sessions.
