Locksmith Codex entry not unlocked despite completing objective

I've included two videos here. One for exfil-ing with a fresh prestige key template, and one for exfil-ing with the key template objective completed. The videos are long in order to show the run, but you can skip to the end to see the key template being exfil-ed. Despite this, the Locksmith codex entry is still not unlocked, shown below:
