Hammer/Anvil challenge didn't track
I played with two randoms on a run on perimeter. The first enemy (I'm not sure if I was the one to down the enemy or a crewmate) I used two heavy melee attacks with my knife to kill the enemy that was downed. Then near the end of the run, a crewmate downed a lone runner (not a rook. The third crewmate from a previous crew we fought). Once the enemy was down, I shot him a couple of times with my gun to weaken him, then I threw a heat grenade. He didn't die from the initial deploy of the grenade, nor the heat procs. Once the heat grenade effect was over, I threw an emp grenade, and that killed him. It even showed the grenade icon in the kill feed. When we left the run, it didn't give me the codex for it.

Thanks for reaching out and for the detailed breakdown of what you did.
For the Hammer/Anvil codex challenge, the tracking is stricter than the description makes it sound. To complete it, in a single run you need to:
Kill two different enemy runners
Kill one runner using only melee, and
Kill another runner using only a grenade
Finishers don’t count, and mixing damage sources on the same runner (for example, gun + grenade or melee + grenade on one target) won’t satisfy the challenge.
In your run, both targets took mixed damage before dying, so it didn’t count even though it feels like it should. If you're sure you were able to satisfy the requirements in your next attempt, please have a screenshot or short clip ready so we can further investigate.
