I didn’t get a recording of me reviving both of my teammates with the Reboot+ ult, but I can send a screenshot of the codex showing that the achievement isn’t marked as complete if that helps at all.
Otherwise I’m not sure what I could provide. I’d say I can just try to record it whenever I can do it again, but no idea when I’ll have the chance, and also unsure that the achievement will actually register then either.
If this issue happens again, would you be able to record it so we can pass it along for investigation.

First time was in Cryo when both of my teammates were fully dead and no enemies were around, this time I rezzed both of my teammates mid-fight with one of them quickly getting knocked again, and the other living a bit before we all got wiped.
