Codex entry not being fulfilled no matter how many times I attempt it

I'm having an issue with the mastery codex entries on assassin and triage, I've attempted the "near death salvation" codex entry numerous times and seemingly completed it only to be met with nothing upon exfil, similarly my teammate who plays triage has seemingly completed the codex entry for reviving two people at once with reboot+ but did not get the reward. are there details we are missing?
