Missing Codex Entry – “For Carnage Apply Within”

Hello,

I was working on tracking the “For Carnage Apply Within” Codex entry by tracking it with CyAc //: Servants of Death in Cryo Archive. I reached 191/200 Destroy UESC Puppets, and my run report showed 111 Combatant Eliminations.

Based on my combatant eliminations, I believe I should have received the Codex entry, but it did not unlock.

Could you please look into this and clarify whether this is a bug or if there are additional requirements I may have missed?

We understand the For Carnage Apply Within Codex entry didn't unlock from your Vault Breaker run. We're sorry to hear of this, as we can imagine how this may have caused concern. Should you experience this again, please reply with the following for our investigation:
- The approximate date you first experienced this issue on.
- A linked videoclip showing you reach the number of kills required upon exfil, and the Codex entry remaining locked.
- Any other specific steps which appear to reproduce this.
Once we have the above information, we'll investigate. Thank you.
