Codex entry did not unlock

- Description of Issue - "Trinity star" codex entry is not unlocking after completing it during run. My filled crew has been eliminated and after solo ace-ing a whole enemy team (they even managed to rez the thirsted player), the codex entry did not unlock, even though technically I have eliminated 4 runners while my team was spectating me.
- Rough Date and Time of issue - march 20th after whole run with filled squad
- Repro steps - I do not know if it is related but my game was in crash-like freeze during loading (sometimes it crashes before a run starts)
- LINKS: Not much, as I do not record my gameplay, but I have got a screenshot of results - 4 runner eliminations and 2 crew revives (only one teamfight) https://imgur.com/a/zMreZlV
