Out of date/inaccurate codex entries
Various codex entries describe behavior for items that has since been changed, or in some cases seem to have placeholder details. For instance in this (I hope it attached correctly :/) screenshot you can see the shield charges still have old text from when they recharged one bar per shield charge. Now shield recharges have been reworked to fully recharge the shield, but do it slowly compared to the advanced shield chargers, as opposed to "fully recharge LEVEL 1 shields." and "partially recharge up to LEVEL # PSDs".
Some codex entries also (such as the frag grenade entry) only have a "#" where it seems they are intended to have specifics, such as the explosive radius.
