While in raid some items will appear with zero value. Not sure if I recognized a pattern but items, even when stacked, will present as zero value.

You aren't using the self revive. You are giving up. You need to hold Y to self revive. Not X to give up. I had this "issue" as well and thought I was getting killed upon self revive but I was hitt...
