Codex Entry Not Unlocking

Platform - Xbox
Description of Issue - Codex>World>Activities>New Cascadia Colony>Decon Data Transfer = Bugged. It's showing as claimable in the codex but won't unlock when clicking on it.
Rough Date and Time of issue - Sometime during season 1, I noticed the issue but never posted a forum for it. Tried completing another contract that used the DCon thinking that may help, the codex entry is still locked and showing as claimable.
I'm also having another issue involving the Early Bird VIP codex where I only received Mida and SekGen rewards, even though I had VIP for all factions prior to the end of season 1. I saw that this was being investigated in other posts, but wanted to still bring it up since I am under that umbrella too.

Bungie Account Name - AlpaccaLipss #9734. Platform - Steam. Description of Issue - posted this in the discord but i will copy here. Does anyone know if Hypocritical Oath requires you to hit a hostile runner or UESC along with your crew member with Reboot+? I have done this twice on UESC and my crewmate and it still has not given me the Codex for Triage to complete it. And all it says is Hit a hostile and a crew member with a single Reboot+ activation.
