Throwable Utility Not Being Used

Issue; I have had multiple times when throwable utility (bubbles and just now with frag grenades) where I will throw a piece of utility by pressing my bind, I will watch it leave my hand and hit the floor, yet it will disappear and return to my inventory like I never used it. 

This happened 2 days ago with the bubble a few times, then just now today around 7:15 P.M. with a frag in Ranked Cryo.

I am unsure of what triggers the bug, but I am not doing anything special. I am simply using my utility and it is not registering as being used. Seems server side to me. I have a clip of it happening with a bubble if that would help here:
