Jurisdictional Annihilation Codex entry not working

I have completed the Jurisdictional Annihilation codex entry by killing a warden with a grenade and it did not show as completed. I feel as if I have done this a few times but I do have video footage of this occurrence. The Warden died to damage from a chem grenade which in the codex entry description should count. If you would like to see the footage I can post a link. Thank you.
