My keys are disappearing

It’s happened twice now 3/20-3/21 where I was extracting with a fragile key and had in my inventory the whole game and after extracting my keys were gone from not only the exfil screen but also gone from my vault completely as if I didn’t have them. Where have my keys gone?!
