PC - MIDA Contract Bug: Order/Chaos, Possibly Bugged Instance?

When running Order/Chaos, we possibly encountered a bugged instance. Didn't think to get screenshots unfortunately.

But when we killed the Commander, their credentials for the Order/Chaos contract did not drop.

Additionally in this instance, we also defeated a USEC unit, and after about a 30 seconds, I saw the death animation play again even though there was no unit to die. Strange behavior.

Otherwise perfectly normal match. If it happens again I'll update here with screenshots.
