Worse performance with VSync on

With it on I would be around 60fps and even when it was stable the picture would be unstable. Settings didn't change much at all, even upscaling.

With it on I get a big increase to around high 70s-80s and top end low 90fps. There is no more stuttering/etc. either.

Something is bugged with the in-game VSync.
